And through your professional association, you can apply for up to $500,000 of coverage at cost-effective rates.

- Accelerated Benefits Option (ABO)*. If you, or your spouse, has been diagnosed with a terminal condition that results in an expected life span of 12 months or less, the Accelerated Benefits Option will pay up to 80.0% of the Insured’s term life amount. Upon death of the insured, the amount of insurance then payable will be reduced by the amount paid under the Accelerated Benefits Option.
- Waiver of Premium Benefit. If you become disabled while insured and before age 60, and the disability last for at least six months, your coverage will continue with no premium due. Your coverage is continued to age 65 as long as you are disabled. The plan premiums will be waived as long as you are disabled.
- Will preparation** and Metlife Estate Resolution Services: automatically included with your coverage.

- All eligible SFSP members and their employee’s working at least 20 hours per week, under the age of 65.
- Unmarried, dependent children between the ages of 15 days and 26 years.
- The member must be insured in order to cover dependents.
- A member can apply for coverage in $10,000 increments, from $20,000 to $500,000.
- Spouse coverage can be applied for in $5,000 increments, from $5,000 to $250,000, not to exceed 100% of the member’s Life Benefit.
- Premiums for both Member and spouse are based on the Member’s age.
- Premiums increase when a member enters a new age band and are adjusted on the premium due date coinciding with or next following this change.
- Coverage reduces in value beginning at age 65. The amount of coverage is reduced by 35% at age 65, 50% at age 70, and 65% at age 75. Rates do not decline.
- All eligible children can be covered for $0.09 per thousand of coverage from age 15 days up to age 26. Coverage is elected in $2,000 increments from $2,000 to $10,000.
- Through Aging. Premiums for members and spouses increases as the member ages and enters a new age band. Also, the coverage amount reduces beginning at age 65.
- Through group policy renewal: As this is a group policy, the insurer will review the policy on each policy anniversary. Rates per age bands can be changed at that time, but only once in a 12-month period.
- Coverage becomes effective on the first of the month following approval of your application by Metropolitan Life Insurance Company. Coverage will not be effective if the member, or spouse and/or your children, if also approved, are confined to a hospital or other medical institution and are not performing normal daily activities on the effective date of coverage.
- Your coverage continues as long as you remain an active member of the association working 20 hours per week. Premiums must be paid when due.
- Dependent coverage ends when they no longer meet the age requirements.
- Benefits are paid to the beneficiary of the member’s choice for death of any cause, except suicide during the first 24 months of coverage (Missouri, North Dakota and Colorado, 12 months).
- The member is automatically the beneficiary of his or her covered dependent children.
- For Group Term Life insurance, in the event of your death by suicide during the first 24 months of coverage (Missouri, North Dakota and Colorado, 12 months) benefits will not be paid. Premiums that have been paid will be refunded to the beneficiary designated.

Accidental Death & Dismemberment insurance does not include payment for any loss which is caused by or contributed to by: physical or mental illness, diagnosis of or treatment of the illness; an infection, unless caused by an external wound accidentally sustained; suicide or attempted suicide; injuring oneself on purpose; the voluntary intake or use by any means of any drug, medication or sedative, unless taken as prescribed by a doctor or an over-the-counter drug taken as directed; voluntary intake of alcohol in combination with any drug, medication or sedative; war, whether declared or undeclared, or act of war, insurrection, rebellion or riot; committing or trying to commit a felony; any poison, fumes or gas, voluntarily taken, administered or absorbed; service in the armed forces of any country or international authority, except the United States National Guard; operating, learning to operate, or serving as a member of a crew of an aircraft; while in any aircraft for the purpose of descent from such aircraft while in flight (except for self-preservation); or operating a vehicle or device while intoxicated as defined by the laws of the jurisdiction in which the accident occurs. - physical or mental illness or infirmity, or the diagnosis or treatment of such illness or infirmity;
- infection, other than infection occurring in an external accidental wound, or from food poisoning;
- suicide or attempted suicide; (In Missouri, such exclusion only applies while the person is sane);
- intentionally self-inflicted injury
- service in the armed forces of any country or international authority. However, service in reserve forces does not constitute service in the armed forces, unless in connection with such reserve service an individual is on active military duty as determined by the applicable military authority other than weekend or summer training. For purposes of this provision reserve forces are defined as reserve forces of any branch of the military of the United States or of any other country or international authority, including but not limited to the National Guard of the United States or the national guard of any other country; any incident related to: 1) travel in an aircraft as a pilot, crew member, flight student or while acting in any capacity other than as a passenger (applies to: Supplemental ADD); 2) travel in an aircraft for the purpose of parachuting or otherwise exiting from such aircraft while it is in flight; 3) parachuting or otherwise exiting from an aircraft while such aircraft is in flight except for self preservation; 4) travel in an aircraft or device used for testing or experimental purposes; by or for any military authority; or for travel or designed for travel beyond the earth’s atmosphere;
- committing or attempting to commit a felony;
- the voluntary intake or use by any means of: 1) any drug, medication or sedative, unless it is: taken or used as prescribed by a Physician, or an “"over the counter”" drug, medication or sedative, taken as directed; 2) alcohol in combination with any drug, medication, or sedative; or 3) poison, gas, or fumes;
- war, whether declared or undeclared; or act of war, insurrection, rebellion, active participation in a riot;
- driving a vehicle or operating another device while intoxicated as defined by the laws of the jurisdiction in which the vehicle or other device was being operated.

Multiply the monthly premiums shown for your age band by the number of $10,000 units you desire (minimum 2 to maximum of 50 units): spouse premiums are determined in the same manner, based on the member’s age.
